【背景知识】

肠内蛋白质丢失是各种全身或局部胃肠道疾病的严重后果。这些通过损害肠粘膜完整性（例如过敏，炎症，恶性变化）或肠道区域的淋巴充血导致血浆蛋白转移到肠腔中增加。这可能导致伴有水肿的低蛋白血症。通过排除其他蛋白质丢失来源和检测到粪便中升高的α，-抗胰蛋白酶浓度来确认诊断。

在血清中，α,-抗胰蛋白酶占丝氨酸蛋白酶抑制剂的大部分，在炎症反应中保护组织免受蛋白酶损伤。它主要由肝脏合成，但也有少量由肠巨噬细胞、单核细胞和肠上皮细胞合成。 α,-抗胰蛋白酶重新 - 对消化酶的降解具有相对抵抗力，它在粪便中几乎没有变化。因此，检测 α,-抗胰蛋白酶是公认的肠蛋白丢失和增加黏膜通透性的标志物。除了测量粪便样本中简单的 24 小时 α,-抗胰蛋白酶排泄量外，还可以测定 α,-抗胰蛋白酶清除率（从日常临床实践中粪便和血清样本的α，-抗胰蛋白酶ELISA值）。 J. S. Fordtran 周围的小组表明，与 α,-抗胰蛋白酶清除率相比，仅测定 α,-抗胰蛋白酶粪便浓度在 21% 的病例中会产生假阳性或假阴性结果（Strygler 等，1990）。

由 Immundiagnostik 开发的 IDKRa,-Antitrypsin-ELISA 在血清、粪便和细胞培养物的分析中明显优于先前使用的径向免疫扩散 (RID)。在直接比较中，ELISA中测定的α，-抗胰蛋白酶值平均比RID的相应值高30%。来自肠细胞系的细胞培养上清液在 RID 中呈阴性，来自几名淋巴阻塞患者的粪便样本也是如此。 ELISA 能够检测到所有这些样品中的α-抗胰蛋白酶，有时甚至检测到非常高的浓度（Faust el a. 2001）。

结果清楚地表明，IDKRa,-antitrypsin-ELISA 的灵敏度明显高于其他常用方法，不仅可以检测肝形式，还可以检测肠内α，-抗胰蛋白酶形式。特别是在肠内蛋白质损失极高的情况下，它明显优于径向免疫扩散。我们的 lDKRa,-抗胰蛋白酶-ELISA 中两种特异性抗体的组合在很大程度上消除了假阴性结果的可能性，从而确保了可靠的诊断。
