【背景知识】

血液中99%以上的甲状腺素（T4）与载体蛋白结合；甲状腺素结合球蛋白（TBG）。然而，只有甲状腺素的游离（未结合）部分才对生物作用负责。此外，在许多临床条件下，载体蛋白的浓度会发生改变，例如怀孕在正常甲状腺功能中，随着载体蛋白浓度的改变，总T4水平发生变化，游离T4浓度保持不变。因此，游离T4浓度的测量与临床状态的相关性比总T4水平更可靠。总T4水平的增加与妊娠、口服避孕药和雌激素治疗相关的水平导致总T4水平升高，而游离T4浓度基本保持不变。
